The story opens after the death of Don Vello Ferrante.
Luca Ferrante becomes the new Don of the Camorra, Achilles serves as Underboss, and twenty-six-year-old Enzo works as the family’s enforcer.
Enzo hides two things from his brothers: a stress-triggered psoriasis condition and his attraction to men in a homophobic organization.
One night Enzo visits a Manhattan gay club under a false name. In a nearby alley, a small-time criminal named Cash steals his switchblade and threatens to expose his sexuality to the Camorra.
Tom Rothwell, a forty-year-old FBI supervisory special agent who has surveilled Enzo for three years, steps out of the dark and snaps Cash’s neck.
Tom orders Enzo to dispose of the body and refuses to explain himself. Enzo follows him, kisses him, and later tracks him down again.
At 26 Federal Plaza, Tom learns that FBI headquarters is shelving the Ferrante case.
To keep it active, he tells his supervisor, Special Agent in Charge Ezekiel Maddox, that he has established contact with Enzo as a potential informant.
Tom begins meeting Enzo under the cover of that lie. During one encounter he notices jagged scarring on Enzo’s wrists and mistakes it for self-harm.
The marks come from psoriasis flares that worsen when Enzo suppresses his emotions. Meanwhile, the Camorra arranges an alliance marriage between Enzo and Katya Rasputin, the twenty-year-old sister of Bratva Pakhan Alexei Rasputin. Enzo and Katya agree on an open marriage, but the wedding deadline increases Enzo’s stress.
Tom eventually breaks his own rules and goes to Enzo’s apartment at night.
After a brief fight, they begin a physical relationship governed by Tom’s conditions: secrecy, exclusivity, and no emotional attachment. Their contact continues through texts and covert meetings.
Tom’s restraint traces back eight years, when his secret lover Matthew married a woman named Iris under family and religious pressure.
Matt continued the affair until his father threatened to expose him to his church, and Matt then died by suicide. Tom responded by building a life of rigid routine to avoid ever harming anyone again.
Enzo follows Tom to Staten Island, sees him with Iris and her son Silas, and assumes Tom is married. Enzo cuts off contact. Tom confronts him and explains Matt’s death, and the confession brings them closer.
Mob politics then escalate. Sofia Bandini leaves her husband Luca to live in the suburbs with Jeremie Rasputin, a Bratva cyber operative, and Luca threatens war.
At a Camorra-Bratva sit-down at the Forbidden Fruit club, Slava Rasputin questions Enzo’s reliability and Alex Rasputin warns that humiliating Katya will mean death. Enzo agrees to keep the engagement if Jeremie is removed from Sofia’s life.

Tom takes Enzo to a lake house in Lake George, nicknames him “Butterfly,” and hears how Enzo secretly poisoned his abusive father Vello. Back in the city, Luca sends Enzo to collect protection money in Chinatown, where Enzo finds Tom dining with FBI colleagues.
Tom ignores him and makes a dismissive remark about mobsters to protect his cover.
Humiliated, Enzo leaves and later kills a convicted predator in his mother’s soundproof basement. When Tom arrives to apologize, Enzo tries to end the affair, but Tom admits that Enzo has become his “life support.”
Slava Rasputin has secretly become an FBI informant while planning to stage a fake Bratva drug shipment at the New York port.
His goal is to bait the Ferrante brothers into an armed confrontation, have the FBI arrest them, and claim his sister Katya for himself.
Tom uses Slava’s operation to try to save Enzo.
He arranges a staged arrest on a minor extortion charge, transfers Enzo to FBI custody, and offers a pre-approved immunity deal that would let Enzo leave the mob, come out publicly, and enter witness protection.
Enzo refuses, because accepting it would abandon Luca and Achilles to prison. He throws the deal at Tom, ends the relationship, and demands a lawyer.
Enzo returns home and cancels the wedding, coming out to Katya as gay. She accepts the truth and agrees to tell Alex that she broke the engagement because she fell in love with her art professor.
Enzo then comes out in front of Luca, Achilles, Lila, Tierney, Tiernan, Sofia, and Tristan, admitting the affair with Tom. His brothers do not reject him, and the family rallies around him.
Tom decides to burn his own career instead. He meets Luca in secret, reveals Slava’s trap at the port, and tells him to keep the Camorra away on Wednesday night. When SWAT and FBI units raid the port, they seize one hundred kilograms of cocaine and arrest fifteen Bratva soldiers.
Slava escapes into woods near the dock, where Enzo intercepts him on a motorcycle. Enzo shoots him in the knee, reveals that Tom was his lover, and spends the following three weeks killing and flaying him in his mother’s basement.
At 26 Federal Plaza, Maddox confronts Tom about the collapsed operation. Tom confesses that he loves Enzo. Maddox instead tells him that Enzo brought him back to life after years of grief over Matt, shields him from criminal charges, and arranges his transfer to the counterterrorism division.
Enzo enters therapy, treats his stress, and his psoriasis clears. He reunites with Tom, and two months later Tom attends a dinner at the Ferrante mansion, where the family accepts him as Enzo’s partner.
In the epilogue, Enzo makes camel-colored leather jackets from Slava’s cured hide and sells them through a luxury boutique.
In the underworld they function as a status symbol and as a quiet warning to vulnerable people and sex workers.
Tom then takes Enzo back to the alley outside the gay club where they met, kneels, and proposes. Enzo says yes.
