The crisis opens at 9:50 AM when Founders Hospital’s main power drops and automated lockdown protocols seal every exterior door and stairwell.
Four armed individuals take positions on separate floors.
On the sixth floor, 25-year-old tech worker Shane Perkins uploads a virus that kills the hospital’s hard-wired phone and internet systems. A Wi-Fi jammer on the fourth floor blocks cell signals, leaving the building almost entirely cut off.
FBI Special Agent Sandra Vos and her partner Special Agent Brice Sutton are dispatched after donor Ilene Maddox calls the FBI Director.
Ilene’s son, neurosurgeon Dr. Jordon Maddox, was on the phone with her on the fourth floor when he saw a gunman.
Sandra arrives at the mobile command center run by MPD Lieutenant Rick Kreiger and Lieutenant Neal Coleman. Kreiger pushes for a tactical breach; Sandra argues for negotiation first.

Sandra reaches Sparling by radio, but he stays guarded and refuses to state demands. Jordon briefly gets hold of Sparling’s gun through Nurse Leah Winters, yet cannot bring himself to fire because of his medical oath.
Sparling then finds a news clipping on Jordon’s phone and realizes he is holding a high-value hostage. Outside the hospital, MPD Detective Eric Birch — Sandra’s boyfriend — begins working the suspect leads.

Eric arrests hospital IT worker Stevie Cross, who admits he let his nephew Shane Perkins into the server room. Eric also interviews Wyatt Beal, husband of hospital CEO Megan Beal, and clears former intern Celeste Barrington of involvement.
As the network comes partly back online, Nurse Maria Torres tells Sandra that Phoebe will die without the transplant. This sets a hard deadline: if the standoff is not resolved by 9:00 PM, the donor heart goes to another recipient.
On the eighth floor, 37-year-old Mindy Ashmore — known on a forum as “Alaya Princess” — holds eleven hospital board members in a boardroom. CEO Megan Beal, executive Murray Berkshire, and transplant surgeon Dr. Valerie Cowan are among them.
Around 2:25 PM, Jordon steps in when Sparling drags Maria Torres into the hallway. Sparling shoots Jordon in the torso.
Maria wraps her lab coat around the wound, accepts his sudden proposal with a family heirloom ring, then shocks Sparling with a defibrillator and locks him in a supply closet.
Eric Birch traces the four assailants through vehicle registrations and library checkout records. They met on an online support forum called “Fat Cats,” where members shared grievances about medical bills, and passed handwritten notes inside tax law library books.
All four suffered catastrophic events at Founders Hospital followed by crushing debt. Mindy Ashmore owes $250,000 while battling recurrent breast cancer after losing her job.
Tom Sparling owes $175,000 after his wife Mable died from a misdiagnosed illness following brain surgery. Carmen Feeney owes $100,000 from her mother’s fatal hospital-acquired infection and faces eviction.
Shane Perkins is grieving his brother Sullivan, whose stroke care left their mother $100,000 in debt across two jobs. At 6:20 PM, Sandra calls Mindy on her husband’s cell phone.
Sandra offers to deliver $250,000 in cash if Mindy orders her team to surrender over the walkie-talkies. Mindy broadcasts the instruction, and Carmen Feeney and Shane Perkins agree to give up without resistance.
Tom Sparling, however, has escaped the supply closet using an internal release latch. He refuses to surrender and takes Gail and Phoebe Chapman hostage in their room.
At 7:05 PM, Sandra carries the cash up eight flights of stairs with an ERT squad. She enters the eighth-floor boardroom alone to hand over the money.
Mindy, in despair, throws cash at CEO Megan Beal and discharges her gun, grazing Beal’s side. When Mindy lifts her handgun to surrender to Sandra, executive assistant Pamela Cherry opens the window blinds.
An ERT sniper outside reads the movement as a threat and shoots Mindy through the window, killing her instantly. Sandra then rushes to the fourth floor for the last standoff.
She brings Sparling’s stepdaughter, Trudy Hall, and $175,000 in cash, and lets Trudy speak through the door. Trudy appeals to the memory of his late wife Mable and urges him not to ruin his life further.
Sparling breaks down, releases Gail and Phoebe, throws his gun into the hall, and surrenders at 8:35 PM. Dr. Valerie Cowan immediately preps Phoebe for the transplant after the donor hospital confirms the organ is still available.
Jordon Maddox and Colby Mahoney survive surgery and are expected to make full recoveries. At Howard University Hospital, Sandra visits her adoptive mother, Margo Davenport, whose fainting spell is diagnosed as anemia from a treatable bleeding peptic ulcer.
Sandra ends the day with her daughter Olivia, Eric Birch, and her family, reflecting on the loss of life inside the hospital. Every Last One closes at 9:15 PM as the lockdown is lifted.
