In Wichita, Kansas, Gina Royal lives an ordinary suburban life with her husband Mel and their two children, ten-year-old Lily and seven-year-old Brady.
She never asks about the padlocked door on Mel’s garage workshop.
When a drunk driver crashes an SUV through that garage wall, police arrest Gina at gunpoint. Detective Salazar shows her what the wreck uncovered: a naked, mutilated woman hanging from a winch hook.
Melvin Royal is exposed as a serial killer who abducted, tortured, and murdered young women in that workshop. Gina is tried as an accessory before being acquitted of any involvement.
Acquittal brings no peace. The public brands her “Melvin’s Little Helper,” and a relentless campaign of doxxing, death threats, and online harassment forces her into hiding.
Four years later, Gina is now Gwen Proctor — her fourth false identity. She lives in a rustic Tennessee community on Stillhouse Lake with her daughter Lanny, fourteen, and son Connor, eleven.

Detective Prester and Officer Lancel Graham question the lakeside residents. Lanny admits she heard a boat engine cut out and a splash on the night of the murder.
Gwen trades her Jeep plus cash to Javier for a cargo van as a bugout vehicle. But watching her children finally make friends, she decides not to run.
A new letter arrives from death row. Mel describes Gwen sitting on her porch in the moonlight — someone is photographing her and feeding him information.
Using a failsafe provided by Absalom, Gwen triggers an attack on Mel in prison. Leaving the kids with Sam, she flies to Kansas and confronts her beaten but arrogant ex-husband, who boasts that his followers will destroy her. She threatens to kill him and returns to Tennessee.
Officer Graham soon pressures Gwen to come to the station without a warrant, waving a fake prop document. Days later, an elderly couple discovers a second mutilated body floating directly in front of Gwen’s house.
At the same moment, a troll forum doxes her Tennessee location. A mob led by Carl Getts smears red paint and slurs across her home, blocks her escape, and corners her on the road.
Gwen swerves cross-country, crashes, and holds off the drunk attackers with a strobe flashlight and a tire iron until police arrive. Detective Prester then interrogates her — and drops a revelation: Sam Cade is the brother of Callie, Mel’s final victim.
Sam had tracked Gwen to force a confession, but her devotion to her children convinced him she was innocent. Prester clears and releases them both.
At Sam’s cabin, a shotgun blast shatters the window. Gwen yanks Sam down a split second before the shot takes his head.
She knows it’s a diversion. Racing home with Sam and Officer Kezia Claremont, they find blood in Connor’s bedroom and the safe room breached. Lanny and Connor are gone.
The pieces form a terrible picture. Officer Lancel Graham is the copycat killer — he cloned Connor’s phone, learned the alarm code, and staged the abductions. Worse, his messages reveal Absalom as a double agent feeding Gwen’s information to Graham on Mel’s behalf.
Graham drives Gwen into the storm-soaked woods. She attacks him inside his SUV, unlocks the door, and flees into the rain — pursued by Graham and his armed teenage son Kyle.
At the ridge, Gwen sets her own ambush. She plays helpless, shoots Graham in the shoulder, and impales him on a sharp wooden branch. She forces him to reveal the hiding place: an isolated hunting cabin up the mountain.
Gwen, Sam, and Kezia hike up and disarm Graham’s sons. Beneath a rug, Gwen finds a trapdoor — and below it, an exact replica of Mel’s Wichita killing floor, complete with winch, noose, and flaying tools. She breaks into a locked cell and finds Lanny and Connor alive.
As police secure the scene, Detective Prester delivers one last blow: Mel Royal has staged a mass breakout from El Dorado and is loose with sixteen other inmates.
Rachel Caine ends Stillhouse Lake on Gwen’s cold resolve — she is done running, and when Mel comes, she intends to kill him.
