Spoiler warning: This section reveals the novel’s plot, including its ending.

Dr. Sean Sullivan attends the second wedding of her hospital colleague, Brandon Pedersen. There, she meets Julian Conrad, the man of honor. They share an immediate connection over a piano.

Julian pursues Sean with an expensive espresso machine and thoughtful dates across Chicago. These include a Turkish restaurant and an art gallery exhibition. Despite their eleven-year age gap, Sean feels drawn to him.

Soon after, Sean begins experiencing severe fatigue, body aches, and fever. Medical tests return normal results. Oddly, her symptoms vanish whenever Julian is present.

Sean confides in Julian about her abusive ex-fiancé, Eric, who abandoned her during a near-fatal miscarriage. Julian shares that his former love, Claire, was murdered decades ago.

Sean faints at a cardiology conference and wakes to find her dark locs have turned silver-white. She cuts her hair short. A medical student notes her symptoms resemble opioid withdrawal.

Sean realizes her crashes align with Julian’s absence. Meanwhile, Brandon collapses and is diagnosed with an aggressive brain tumor. He has been having visions of his wife, Leila.

Old photographs show Brandon aged rapidly after meeting Leila. In Die for Me, Sean concludes that Julian and Leila are immortal beings who drain human life force.

Leila has been killing Brandon. Julian has unintentionally siphoned years from Sean. When Sean confronts Julian, he ends their relationship to protect her.

Sean visits an art exhibition by Rose Callahan, another immortal. A former victim attacks Rose. Rose drains his life in seconds, aging him into a corpse.

Leila orders her henchman to execute the gallery witnesses. Julian rescues Sean and flees to a remote cottage. He arranges to surrender his fortune to Leila for Sean’s safety.

Leila tracks them and takes Sean’s friends hostage. At a cliff at sunrise, Sean stabs Leila with a glass shard. Julian kills Leila’s henchman, Robbie.

Rose performs a reverse life-transfer, turning Leila into an infant. Rose sacrifices her own youth to raise her. The threat is neutralized.

Sean tells Julian she cannot be his only reason for choosing mortality. She insists he must embrace humanity for himself. They part ways.

Five years later, Sean lives in North Shore, Massachusetts. She has a new dog and a peaceful life. Julian arrives at her door, now fully mortal by his own choice. Shirlene Obuobi closes the novel with Sean welcoming him home.
