Psychotic Obsession Summary

Spoiler warning: this section covers the whole plot, including the ending.

Aria Miller, 27, travels from Scotland to Florida with her best friend Gabriella to organize clinical trials for Ivy Dermot. At the hospital, Aria trips over an outstretched leg and meets Tobias Mitchell, a 26-year-old assistant with striking blue eyes. He works under Dr. Blythe, who is secretly his stepfather.

Tobias fixates on Aria at once and hires a private investigator to dig into her life. He learns she recently took a break from Ewan McElroy, her partner of eight years, and he decides to remove Ewan from the picture.

During a night out, Tobias sees his co-assistant Justin spike Gabriella’s drink. He stops the attack and drives the two women to their hotel. There he secretly drugs their bottled water with strong sleep medication so he can lie beside an unconscious Aria.

Aria tries to keep things professional, but Tobias pursues her relentlessly. Their encounters escalate in hotel rooms, cars, and hospital spaces. Unknown to her, Tobias flushes his own five daily psychiatric medications, including Clozapine, and begins dosing Aria’s water, coffee, and wine with it.

Aria develops hallucinations, disorientation, and emotional swings. Lucy, a terminally ill patient Tobias had befriended, dies, and Ivy’s trial hits administrative setbacks. Aria returns to Scotland, planning to end the affair.

Tobias follows her. He rents a nearby apartment, installs hidden cameras in her home, and interrupts her presentation on RPI deficiency. When Ewan visits, Tobias confronts him and declares himself Aria’s boyfriend.

As the Clozapine builds up, Aria suffers panic attacks. She wanders barefoot in freezing rain, convinced she must save Jason from a threat that does not exist, and collapses. Blood tests show dangerously high Clozapine levels, and a scan reveals she is pregnant with twins. Ewan and Gabriella realize Tobias has been drugging her.

Tobias lures Aria out of the hospital and plans to take her to Ireland. She texts Gabriella her location, and armed police raid the hotel room. Tobias fights the officers before being tasered and arrested.

In custody, Tobias kills a guard and his cellmate and escapes with Justin’s help. He then murders a facility worker and burns down a wooden cabin. With Justin and his mother Violetta, he captures Aria, Gabriella, and Kaleb and takes them to an abandoned animal shelter.

Out of jealousy, Tobias slashes Kaleb’s throat in front of Aria. When Gabriella screams, Justin stabs Aria in the side. Aria grabs the knife and stabs Justin repeatedly until he dies.

Tobias drags Aria through snowy woods toward a cliff as police close in. Aware that he is a danger to his unborn children, he considers jumping. Aria persuades him to surrender, and police tackle him.

Tobias is declared unfit to stand trial because of severe mental illness and is committed to a high-security psychiatric facility. Five years later, Aria is an award-winning pediatric scientist and mental health advocate. She lives with Ewan, who co-parents her 5-year-old twins, Kade and Luciella.

The family makes a supervised visit to Tobias. Calm and medicated, he thanks Ewan for raising his children safely, while quietly keeping his love for Aria.
