Last Patient of the Night Summary: An AJ Docker Thriller
TL;DR: A Wrist Fracture, Then a Body at the ER Door
Dr. AJ Docker works the night shift at Ben Taub Hospital in Houston, where trauma cases arrive faster than anyone can count them.
One evening he treats a nervous young woman for a minor wrist fracture, senses she is in danger, and offers help she refuses.
Hours later, that same woman is dumped outside his emergency room, beaten and dying.
Last Patient of the Night by Gary Gerlacher follows Docker as he tries to give her back a name and find the people who killed her.
- Who: Dr. AJ Docker, an ER co-director and adrenaline-driven physician in Houston, plus his friend Officer Tom Nocal and a police dog named Banshee.

- The payoff: A hidden password, hundreds of millions in cryptocurrency, a massive reward, and a new foundation named for the victim.
- Tone: Gritty, sarcastic, darkly funny, and built around emergency-room banter between violent set pieces.

Last Patient of the Night Summary: What Happens in the Book
Spoiler warning: the following section describes the plot, including the ending.
The novel opens in Jackson Hole, Wyoming, where ER physician Dr. AJ Docker is on a ski vacation.
When a novice skier crashes into an aspen tree and crushes his own throat, Docker performs an emergency cricothyroidotomy with a scalpel and ski gear, saving the man’s life.
Back in Houston, Docker returns to his role as co-director of the Ben Taub emergency department alongside Dr. Deb. He runs the chaotic floor with Charge Nurse Jean, handing out donuts and trading dark humor while treating shootings and stabbings.
He immediately clashes with hospital Vice President Lou Gallagher, who wants higher patient volume to hit profit targets. Gallagher also locks the ER thermostat at a sweltering 74 degrees, and Docker pays maintenance staff with football tickets to secretly rewire it.
During a shift, Docker treats a 22-year-old woman who gives her name as Tracy Palmer and complains of a hairline wrist fracture.
He notices a fresh tattoo of a sorceress casting a spell, with a jumble of letters and numbers worked into the design.
Suspecting she is hiding an abusive situation, Docker and his friend, Houston police officer Tom Nocal, offer to help her. She insists she is fine and walks out of the ER.
Just before midnight, an unknown driver drops the same woman outside the emergency room doors. She has been severely beaten and tortured, and the tips of two fingers have been amputated and burned.
Fifteen minutes of aggressive resuscitation fails, and she dies. Forensic pathologist Dr. Morquist Levy later finds she died from a ruptured arterial venous malformation in her brain triggered by sheer terror.
Docker recognizes the sorceress tattoo and realizes the woman was his last patient of the night. Distraught that she died alone without anyone knowing her real name, he vows to find her killers.
Docker and Nocal visit Finnegan’s Tattoo Parlor, where artist Anne identifies the victim as “Jen” and mentions she danced at a club called The U in East Houston. The club is run by violent Ukrainians.
The two men infiltrate The U, and Docker speaks with a dancer named Linda in a private cubby. Linda reveals that the woman was Jennifer Smithton, an orphan from Arkansas who came to Houston for a better life and became trapped by the club’s boss, known as Dyyavola, Ukrainian for “The Devil.”

As they leave, Docker kicks a 300-pound bouncer named Bohdan in the groin to escape. Furious, Dyyavola orders Bohdan to eliminate Docker.
Meanwhile, homicide detectives Lenny Newsome and Jane Ormund receive an anonymous tip, placed by Nurse Jean at Docker’s request, and raid Jenny’s ransacked apartment. The state of the rooms shows her killers were searching for something specific.
Bohdan and a second gunman later ambush Docker while he jogs in his neighborhood with Banshee, Nocal’s police dog. Docker shoots Bohdan, and Banshee tackles the second gunman but takes three rounds to his Kevlar vest.
Docker’s goofy neighbor Carl walks out with a pistol and kills the second shooter. Docker then performs an emergency chest needle decompression on Banshee, saving the dog’s life.
Carl turns out to be a retired Army Sergeant First Class and Congressional Medal of Honor recipient. He tells Docker they fought together that day and are now brothers.
The mob retaliates again, sending two hitmen named Oleksander and Mykta after Docker in 800-horsepower Charger Hellcats. Docker uses evasive driving along Braeswood Bayou, and Oleksander crashes into the 30-foot concrete channel at 100 mph and explodes on impact.
Mykta chases Docker to the Elgin police station, where officers surround him. He opens fire and is killed in a hail of roughly 130 rounds of police gunfire.
Seeking answers, Docker and Nocal enlist a 21-year-old hacker known as The BT, who traces Jenny’s escort appointments to Giovanni Romanelli, a financial executive staying at the Four Seasons. Romanelli confesses that Jenny infected his corporate computer with ransomware during their meeting, forcing his firm to pay a $2 million Bitcoin ransom.
Detectives Newsome and Ormund join FBI Special Agents Keller and Hixon on the case. The agents trace Dyyavola’s offshore accounts to an Eastern European Bitcoin wallet holding more than 20,000 Bitcoins, worth $1.2 billion, built from years of corporate extortion.
Docker sneaks into an abandoned warehouse next to The U and finds a hidden tunnel leading to a reinforced steel safe-room office. He hides inside a commercial pizza oven while armed men search the building.
When he tries to reach his rental car, the men capture him, but Docker manages to speed-dial Nocal and leave the line open to broadcast his location. He is then brought before Dyyavola in the steel room.
Dyyavola boasts about his $1.2 billion Bitcoin empire and reveals that Jenny stole 10,000 Bitcoins, worth $600 million, from him. He tortured her for the password, but she died of the brain hemorrhage before giving it up.
Dyyavola prepares to perform a “Ukrainian manicure,” chiseling off Docker’s finger joints. Docker uses a thin file hidden in his watchband to cut his zip ties, headbutts Dyyavola hard enough to shatter his skull and cheekbone, and locks in a carotid sleeper hold for four minutes until the mob boss dies.
Dyyavola’s computer session is about to expire with $613 million in Bitcoin accessible by thumbprint. Docker uses a hammer and chisel to sever the dead man’s thumb, scans it, and transfers the funds to his own crypto account seconds before logout.
SWAT raids the warehouse and arrests the remaining gang members. Docker then realizes the tattoo on Jenny’s arm, read in reverse, is the password to her stolen $600 million wallet.
He and Nocal unlock her account and surrender the extortion funds to FBI Agent Keller. The FBI awards Docker an $81.5 million tax-free reward for recovering the Bitcoin, plus $9 million as a whistleblower after he exposes a $30 million hospital billing fraud scheme run by VP Lou Gallagher.
Docker spreads the money widely. He sets up $8.15 million trust funds each for Nocal, IHOP waitress Gladys, and cook Little D, creates a $16 million foundation for human trafficking survivors, and funds $4.5 million hospital hardship accounts for Nurse Jean and Dr. Deb.
He organizes a large public funeral for Jenny and establishes The Jenny Smithton Foundation in her memory. Nocal gives the fully recovered Banshee to Docker as a companion.
Docker resigns from Ben Taub Hospital and drives off toward Montana with Banshee to begin his next chapter. The story continues in later entries of the AJ Docker and Banshee series by Gary Gerlacher.
