The novel opens with a prologue inside the Haskell home. An unnamed hitman corners Angela Haskell while her husband is away and gives her an ultimatum: die by staged suicide, or watch her infant son strangled with a garrote wire.

Angela chooses her child. She says goodbye to Andrew and walks into the garage to die.

Two years later, Mitch Haskell is a decorated former Marine and ex-DEA undercover agent working homicide in Auclair, Louisiana. The official ruling on Angela’s death was suicide, but he refuses to accept it.

To force his way into a covert investigation, Mitch stages a drunken brawl at a local bar and gets himself thrown into a holding tank. His superior and best friend, Lieutenant John Bowie, responds by mandating psychological evaluation.

Mitch picks Dr. Dylan Reede from the approved list on purpose. Surveillance told him she treats Roland Malone, a high-level lieutenant in a regional drug cartel run by a figure known only as “Oz.”

In session, Mitch is combative and openly flirtatious. He kisses her, rattling her composure, then later picks the locks of her office to scout her files and warn her he is close to violence.

Meanwhile, police pull two bodies from Bayou Coeur: a low-level criminal named Paul Adler and an unidentified teenage runaway. In disguise as a homeless man on Esplanade Avenue in New Orleans, Mitch watches Malone’s upscale restaurant, a front for Oz’s distribution network.

Malone leans on a feral young hitman called “El Paso,” whose real name is David Rodriguez, a member of the Mexican Caballeros cartel working inside Oz’s ring.

After dining at Malone’s restaurant, Dylan leaves just as El Paso attacks the disguised Mitch in a boulevard median. The hitman draws a switchblade, and Mitch fights back, taking a slash across the abdomen.

Spotting Dylan in the commotion, Mitch grabs her arm and pulls her into the alleys so she is not identified. He refuses a hospital because a reported knife wound would destroy his cover.

Instead, he has her buy bandages and antiseptic, then drives her to John Bowie’s remote bayou cabin. There she cleans and closes the wound. Mitch tells her everything: Malone works for Oz, she treats Malone, and he believes Angela was killed in retaliation for his old drug busts.

Dylan refuses to breach patient confidentiality, but the tension between them breaks. She gives in, and they sleep together.

The next morning Bowie calls with news. A courier delivered a severed pinkie finger to the police department, addressed to Mitch, still wearing Roland Malone’s signature red stone ring. El Paso had butchered Malone in his own meat locker on Oz’s orders because the restaurant incident drew police attention.

At this point the full plan surfaces. Mitch’s public breakdown and his shouting matches with Bowie were a staged hoax. The two men had been working together all along to make Mitch look unstable and bait the syndicate into mistakes.

Detective Barbara Nix is then exposed as the mole leaking police intelligence to Malone and Oz.

Worried for Mitch, Dylan returns to her office to review Malone’s psychological files for clues about Oz. El Paso ambushes her there with a blade at her throat and demands she bypass the computer firewalls.

A patrolman tells Mitch that Dylan was seen entering the building, and he races over. He ambushes El Paso, pins him, and Dylan slams a desk drawer on the hitman’s fingers so Mitch can disarm and arrest him.

Using Nix’s phone and interrogating El Paso, Mitch and Bowie identify Oz as Allen Busby, a famous television personality known as the “King of Cash.”

Busby used his wealth and fame to hide his role as the region’s chief drug kingpin. Years earlier, he ordered Angela’s murder to punish Mitch for crippling his supply lines.

Federal agent Jim Tucker coordinates a raid on Busby’s incoming drug shipment. Mitch corners Busby in his television recording studio, strips off the mustache disguise, and arrests him.

With Busby in custody, the syndicate dismantled, and Angela’s murder solved, Mitch is released from the obsession that drove him. John Bowie’s wife Beth gives birth to a healthy boy.

Mitch reunites with Dylan at John’s home. He thanks her, lets go of his self-blame, and tells her he loves her as they look ahead to a life together with Andrew.
