Painted Scars Summary: An Enemies to Lovers Mafia Romance

Introduction

Nina Grey has no place in the criminal underworld. She is a twenty-four-year-old painter who graduated from the Art Institute of Chicago and spends her days mixing oils, not dodging bullets.

That changes when her father confesses he has lost three million dollars of Russian Bratva money in a Ponzi scheme.

In Painted Scars by Neva Altaj, the first book in the Perfectly Imperfect series, Nina is offered a trade: six months as the wife of Roman Petrov, the Chicago Pakhan recovering from a car bomb, in exchange for her father’s life.

The novel follows the arrangement from both points of view.

  • The reckoning: The traitor inside the Bratva is unmasked, and Nina witnesses exactly what Roman is capable of when someone threatens her.
  • Central themes: physical and emotional scarring, loyalty against betrayal, and the cost of loving someone whose world runs on violence.

Painted Scars Summary: The Full Story

SPOILERS AHEAD. What follows covers the full plot, including the ending.

The book opens in a Chicago hospital. Roman Petrov, Pakhan of the Russian Bratva, is sedated after surviving a car bomb planted by someone inside his own organization. The blast destroyed his right knee and left him in a wheelchair.

Roman and his second-in-command, Maxim, know that visible weakness invites rebellion. They need a show of normalcy, and a wife suggests a man planning for an heir.

Elsewhere, Samuel Grey has embezzled three million dollars from the Bratva and lost all of it. Facing execution, he tells his daughter Nina everything. Maxim proposes a compromise instead of a killing: Nina marries Roman for six months.

Nina meets Roman in a closed restaurant and, despite her fear, negotiates her father’s survival with a composed, defiant manner. The contract is signed.

She moves into Roman’s fortified estate outside Chicago and takes up residence in his private wing. To protect his image while his leg heals in secret, Nina deliberately performs the role of an eccentric, harmless wife around enforcers and staff.

The act gives her freedom to move through the house. Working with Maxim, she helps plant listening devices in key rooms to expose whoever ordered the bombing.

Roman undergoes physical therapy with Warren, progressing from wheelchair to crutches to a cane. Meanwhile, the attraction between him and Nina stops being performance. He is drawn to her wit and her dark paintings; she begins to see his protectiveness and his sense of honor.

Nina eventually discloses a painful history: years earlier, a man named Brian attempted to assault her, and she fought him off. Roman promises she will never be harmed again.

The pressure escalates when Roman hosts Tanush, a volatile Albanian mob boss, for dinner. When the conversation turns toward bloodshed, Nina walks into the room and defuses it with chatter, charming Tanush and preventing a war.

Hidden recordings then reveal the truth. Roman’s own uncle, Leonid, arranged the car bomb and conspired with Tanush to take his title. Roman confronts Leonid, shoots him in the knee, and executes him.

After a night at Roman’s club, Ural, where he walks in public without a wheelchair for the first time, Nina confesses her love. Roman declares the six-month deal permanently void and tells her he will never let her go.

That same night, Nina spots Brian at the club. Roman reads her terror, identifies the man, and quietly orders his men to take him.

Late that night, Nina wakes to an empty bed and follows a trail to the soundproofed basement. She opens the door to find Roman torturing Brian to death.

Horrified by the violence and crushed by guilt that a man died because of her, she refuses to hear explanations and flees the estate by taxi to her apartment.

Roman does not accept the breakup. When Nina sends signed divorce papers, he returns them with a large “NO” written in red ink across every page, and posts discreet guards outside her building.

He eventually sends a final letter promising not to force her back, but leaving his door open. Without her, he spirals, deliberately provoking a war with Italian families and fighting on the front lines in warehouse shootouts.

Roman takes a bullet to the arm. Fearing he will get himself killed, the housekeeper Varya visits Nina and tells her about his childhood.

As a boy, Roman watched his abusive father, Lev, beat his mother to death. Years later, he killed Lev to stop him from choking Varya.

Varya explains that killing Brian was an ingrained response to remove any threat to someone Roman loves, and that losing Nina has destroyed his will to stay safe.

Nina returns to the estate. She confronts Roman in his office and demands he end the war with the Italians.

Leaning on his cane, Roman drops to his functional knee and proposes with their original wedding rings, promising to protect her and stop the violence. Nina accepts, forgiving him while setting firm boundaries.

Two months later, as they prepare for a formal church wedding, Nina reveals she is pregnant. Roman embraces her, vowing love and protection over his wife and their unborn child.
