In Heart-Shaped Box, Joe Hill introduces Judas Coyne, a retired death-metal singer who buys a haunted pinstripe suit from an online auction. He expects another macabre collectible. The suit arrives in a black heart-shaped box and carries an old man’s ghost.
Craddock McDermott appears with black scribbled eyes and a razor pendulum. He drives Jude’s assistant Danny Wooten to hang himself. Then he possesses Georgia, Jude’s young girlfriend, and tries to force her to shoot herself.
Jude discovers that sharp physical pain interrupts the ghost’s hypnosis. He stabs his palm with a broken platinum record shard and releases his German shepherds. The dogs’ shadow twins attack Craddock and buy time for escape.
Jude and Georgia flee south, but Craddock follows in a phantom truck. At a Denny’s in Virginia, he uses a stranger’s electronic larynx to command their deaths. Georgia nearly slashes her throat with a mirror shard before Jude breaks the trance.
At Georgia’s grandmother’s house, a Ouija board summons Anna, Jude’s former girlfriend. Anna reveals that Craddock sexually abused her and her sister. He murdered Anna and framed it as suicide. She offers to become a door to stop him.
In Florida, Craddock’s living accomplice Jessica confronts them. Her daughter Reese shoots Jude’s dog Bon and blows off Jude’s right index finger. The survivors escape to Louisiana with Angus, the other dog.
Jude’s estranged father Martin dies, and Craddock possesses the corpse. He slashes Georgia’s throat, and she bleeds out. As she dies, Georgia tells Jude to draw a door in her blood.
Jude draws a doorway and doorknob on the floor with Georgia’s blood. Anna’s spirit rises from the body and drags Craddock into blinding white light. The shadow-dogs pull him down, sealing him away. Jude wakes in a hospital. Georgia survives.
Joe Hill’s epilogue shows Jude and Marybeth married back in New York. They adopt puppies and later shelter Reese after her mother goes to prison.
